By accessing or using the Wellbeing Solutions LLC Patient Portal ("Service"), you agree to be bound by these Terms of Service. If you do not agree, please do not use the Service.
Wellbeing Solutions LLC provides a secure digital platform enabling patients to schedule appointments, communicate with care providers, submit medical forms, manage insurance information, and process payments for services rendered.
Appointments must be cancelled at least 24 hours in advance. Late cancellations or no-shows may be subject to a fee of up to $50. Repeated no-shows may result in restrictions on future booking.
Copays and outstanding balances are due at the time of service. By providing payment information, you authorize Wellbeing Solutions LLC to charge applicable amounts. All payments are processed securely through Square.
This portal is NOT for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.
Wellbeing Solutions LLC is not liable for any indirect, incidental, or consequential damages arising from your use of the portal. Our liability is limited to the amount paid for services in the past 30 days.
We may update these Terms at any time. Continued use of the portal after changes constitutes acceptance of the new Terms.
